To make it easier for you I have provided links to my intake forms which may be printed prior to your appointment. Intakes MUST be done prior to your appointment. No worries. If you do not print them and bring them along, the intake can be completed upon arrival. Physician's Permission forms are only for clients who may have medical conditions contraindicated for massage and will need a physician's clearance.
